The following instructions are working great for my Linksys PAP2T, which connects to Gizmo5 and Google Voice.
I was lucky enough to sign up for Gizmo5 before they were acquired by Google and have been using it to connect out from Google Voice for a while. I’m having problems with initiating calls to my VoIP line from Google Voice, but outbound calls don’t seem to have problems. I think that there are integration problems (as of this writing in early March, 2010) because I think the Gizmo5 servers moved to a Google datacenter recently.

It involved a bit of trial and error to get things working, but this configuration appears to be very solid.
Edit (2010-06-11): I changed the DTMF Tx Method to AVT. When set to InBand, I was unable to access several conference call bridges and other phone systems that relied on DTMF. Since making that change, all has worked well.
